Enrolments

East Butler Primary School welcomes enrolments from all students into our community.

Children residing within the school's boundary and enrolling into Pre-Primary through to Year 6 are guaranteed a place at our school. We are happy to receive enrolment applications from out of school's boundary, however acceptance of the application is dependent on the availability of places at that time.

ENROLMENT PROCESS

If you wish to enrol your child at East Butler Primary School please complete/submit the required forms and documents listed below to: eastbutler.ps@education.wa.edu.au. Alternatively, you can bring a hard copy into the school office during school hours.

Please download and complete the following forms to enrol:

• Enrolment Form
• Consent Form

Please also provide the following required documents:

• Birth Certificate (original or certified copy) or Birth Extract
• Immunisation Certificate
• Proof of your usual place of residence (e.g. a utilities account or Lease Agreement)
• Family Court Orders or Parenting Plans registered with the Family Court (if applicable)

Once your application is accepted an Enrolment card will need to be completed along with medical forms and permission forms.
